Panama hosts one of Latin America's largest US veteran-retiree communities. This guide covers every federal benefit that follows you south — retired pay, VA compensation and pension, TRICARE, the Foreign Medical Program (FMP), CHAMPVA, insurance, burial — with verified 2026 numbers and an honest account of the FMP fraud crisis.

Why are veterans retiring to Panama?

The income stack. Three federal streams, all depositable into a Panamanian bank in US dollars — no currency-conversion loss:

Scroll the table sideways to see every column

Income stream Payable in Panama? How to set it up
Military retired pay (DFAS) Yes — Panama is on DFAS's official International Direct Deposit (IDD) country list Form SF 1199-I via AskDFAS upload; allow 45–60 days
VA disability compensation / pension Yes — 38 CFR § 3.653 bars payment only to enemy-territory aliens and Treasury-list countries; Panama is neither VA Form 24-0296A, +1-918-781-7550, DIRECTD.VBAMUS@VA.GOV, or the embassy FBU
Social Security Yes SSA International Direct Deposit (guide 09)

The Pensionado visa. Military retired pay and VA disability compensation count as pension income provided they are lifetime payments totaling at least US$1,000/month — no minimum age, permanent residency from day one (full mechanics: guide 05). Residency unlocks the Ley 6 discounts (20% off medicines and doctor visits, 15% off hospital bills), which stack with US coverage.

The community. VFW Post 3835 "George Washington Goethals" (Panama City) and American Legion Post PC-02 (Nueva Gorgona/Coronado — the only American Legion post in Latin America) anchor veteran life; PC-02's two VA-accredited Veteran Service Officers file VA claims free for any veteran, no membership required, and Boquete Veteran Outreach (+507 203-1033) covers the highlands. VA tracks roughly 1,030–1,100 veterans in-country; community estimates reach ~5,000 with family. Corozal American Cemetery hosts official Memorial Day and Veterans Day ceremonies and is one of only two ABMC cemeteries still accepting new burials — open to US veterans (and certain family members) resident in Panama. ⚠️ Reconfirm the eligibility wording at abmc.gov/Corozal before basing plans on it.

How does TRICARE work in Panama?

Panama sits in the TRICARE Overseas Program (TOP) "Latin America and Canada" region, administered by International SOS (ISOS); toll-free from Panama 008000442028. Panama is one of only two countries named in the TRICARE Reimbursement Manual's overseas inpatient per-diem system.

Retirees get TRICARE Select Overseas — and only that. TOP Prime / TOP Prime Remote are for active duty and command-sponsored families; retirees are not eligible. Select Overseas is self-managed — no assigned doctor, no referrals in most cases — but you pay up front and file claims. Enrollment is required (milConnect, the TOP call center, or DD Form 3043); DEERS must show your Panama address.

Scroll the table sideways to see every column

TRICARE Select Overseas, Group A retiree (CY2026) Individual Family
Annual enrollment fee $186.96 $375
Annual deductible $150 $300
Non-network cost-share (nearly all Panama care) 25% 25%
Annual catastrophic cap $4,381 ($3,000 survivors/medically retired) same

Panama cash prices are low (GP $25–60, specialist $60–120), so many households stay under the deductible — Select Overseas works as cheap catastrophic cover over discounted cash care.

Claims discipline is the survival skill. File online at portal.tricare-overseas.com (set up direct deposit) or DD Form 2642 to PO Box 7985, Madison, WI 53707-7985. Proof of payment is mandatory on every overseas claim; deadline 3 years; most claims pay in ~30 days; translation is free. Demand an itemized factura stamped "pagado" every visit. Local insurance pays first — TRICARE is always last payer.

What happens to my TRICARE at age 65? (the trap)

The single most expensive mistake a veteran can make in Panama:

  • TOP eligibility ends the last day of the month before your 65th-birthday month — individually, per person. A younger spouse stays on Select Overseas until their own 65th.
  • Eligibility at 65 depends on Medicare entitlement and individual circumstances. If eligible for premium-free Part A, Part B is generally required to retain TRICARE; exceptions apply. Confirm with TRICARE before changing coverage. See the linked TRICARE clarification above.
  • The 2026 standard Part B premium is $202.90/month — treat it as the premium that keeps TRICARE For Life (TFL) alive. Sign up no later than 2 months before your birthday month.
  • TFL starts automatically — no enrollment, no fee, no card. Overseas, TFL flips to PRIMARY payer with Select-like exposure: deductible + ~25% cost-shares + charges above the TRICARE-allowable amount, capped at $3,000/year (Group A). File with ISOS, not Medicare — neither Medicare nor the stateside contractor processes overseas claims.
  • Medicare Advantage does not automatically break TRICARE For Life coordination. Confirm plan residency requirements and coordination with TRICARE before enrolling or changing coverage.

Which Panama hospitals accept TRICARE and VA benefits?

Scroll the table sideways to see every column

Facility Where What it accepts Status
Hospital Pacífica Salud (Johns Hopkins-affiliated) Punta Pacífica + Costa del Este, PC TRICARE, CHAMPVA, FMP, direct billing; Veteran Services Coordinator veteranservices@pacificasalud.com; pharmacy meds not covered; some services need pre-approval ⚠️ Announced May 2025 via relocation press — verify directly; worst case pay-and-file
Clínica San Fernando Coronado Coronado beach corridor TRICARE + FMP via current benefit letter to tricarefmp@hospitalsanfernando.com; specialists physician-dependent; complex care routes to PC (6674-4977); CHAMPVA "not yet" Verified on hospital site Oct 2026 — a clinic, not a hospital (no ICU)
Hospital San Fernando Panama City TRICARE/FMP direct billing Practitioner-verified 2026
Hospital Nacional, Hospital Santa Fe Panama City FMP direct billing Practitioner list (Jul 2026) — reconfirm
Centro Médico Mae Lewis David TRICARE and VA FMP; owned since 2025 by Grupo Montecristo (Hospital Metropolitano, Costa Rica); phone 775-4616 Confirmed by phone with the hospital (site owner), Oct 2026
Hospital Chiriquí David FMP direct billing Practitioner list (Jul 2026) — reconfirm

⚠️ Provider lists are fluid post-crisis (below): check va.gov's FMP alerts page and confirm direct billing before planned care.

How do prescriptions, dental, and vision work?

Pharmacy reality. TRICARE Pharmacy Home Delivery cannot ship to a private Panama address (APO/FPO or embassy assignment only), and no network pharmacies exist outside the US: pay full price, keep proof of payment, file with ISOS — reimbursed after the deductible at a 25% cost-share; OTC products not covered. Your Pensionado 20% medication discount stacks first at the register; TRICARE reimburses only what you actually paid (⚠️ interaction inferred from claims rules). Stock 90-day fills on US trips ($0 at military pharmacies).

Dental and vision (FEDVIP). Retirees and families enroll at BENEFEDS.gov (vision requires a TRICARE health plan — TFL counts). Abroad it is pay-and-file at out-of-network rates, with one standout: MetLife Federal Dental pays international care at in-network benefit levels via AXA Assistance (866-384-2771). Panama prices (cleaning $40–60, crown $500–800) often sit below US allowances.

Which VA benefits continue while living in Panama?

  • Disability compensation — yes, in full. Identical rates (2026: 100% single = $3,938.58/month; 50% = $1,132.90); no reduction solely for moving abroad, though non-permanent ratings still face routine reviews. Keep both Panama addresses current with VA — undelivered mail is the leading cause of missed deadlines and overpayment debts abroad.
  • C&P exams happen in-country via VA contractors: call +1-713-255-5656 (caller ID "VA EXAM-VES"). Scheduling is slower, VA won't fly you back, and a missed exam can delay or deny a claim — answer emails fast. Private DBQs from Panamanian physicians are accepted.
  • Dependent verification is not annual. VA Form 21-0538 arrives on roughly an 8-year automatic cycle with a 60-day response window; phone verification (1-800-827-1000 or +1-412-395-6272) counts when mail is slow. Non-response removes dependents and creates debt. The old annual TDIU questionnaire (21-4140) has ended — VA now uses an SSA wage match.
  • VA pension is payable abroad. The "US residents only" blog claim is wrong for US citizens — § 3.653 controls, and VA's handbook lists pension among benefits "generally payable overseas." Income/net-worth limits still apply.
  • SGLI/VGLI stay valid abroad — 38 CFR Part 9 contains no geographic exclusion; premiums by DFAS allotment; OSGLI 800-419-1473. ⚠️ Medium confidence: no VA page states this verbatim.
  • VA home loan — no. VA-guaranteed loans require property in the US, its territories, or possessions (Pamphlet 26-7). But Specially Adapted Housing (SAH/SHA) grants CAN be applied to a Panama home for qualifying service-connected disabilities.
  • Burial benefits travel too. Up to $2,000 for a service-connected death (no deadline); non-service-connected burial/plot allowances (2-year deadline; Form 21P-530EZ); a free NCA headstone for a veteran's grave "in any cemetery around the world" (Form 40-1330; family pays customs/setting); a burial flag via Form 27-2008 at US embassies. The embassy FBU initiates survivor/burial claims: Panama-FBU@state.gov, subject "FB-VA Death Benefits – FULL NAME," (507) 317-5000.

How does the VA Foreign Medical Program (FMP) work in Panama?

FMP pays for care on foreign soil, only for VA-rated service-connected conditions (plus aggravating conditions and VR&E plans) — not general VA healthcare abroad.

  • Register with VA Form 10-7959f-1 (no VA healthcare enrollment required). Your FMP benefits letter — downloadable from VA.gov — is what Panama hospitals ask for before direct billing.
  • Claims: VA Form 10-7959f-2 with itemized bills and proof of payment; online filing since March 30, 2026; 2-year deadline; free translation; medications must be FDA-approved; payment in USD.
  • Direct payment: if the provider files, VA pays the provider directly — no pre-authorization for service-connected care. Otherwise pay and claim. No "FMP network" exists; any licensed Panamanian provider can be paid.
  • Contacts: 833-930-0816 (VoIP from Panama), +1-303-331-7590, fax 303-331-7803, hac.fmp@va.gov, Ask VA.

CHAMPVA abroad: covered under 38 CFR § 17.275(m) with the same $50/$100 deductible, 25% cost-share, and $3,000 cap — but no Panama provider bills CHAMPVA directly; pay and file. ⚠️ The CHAMPVA deadline is 1 YEAR from service, not 3 (the "3-year" figure online is TRICARE's rule; VA counts date of receipt — file online or fax 303-331-7808). At 65+, keep paying Part B to keep CHAMPVA; Meds by Mail does not ship overseas.

⚠️ What is the VA FMP Panama fraud crisis — and how do I protect myself?

The defining veteran-healthcare event in Panama; every provider list published before 2024 is now suspect:

  • August 2024: VA suspended a group of 36 Panamanian medical providers from FMP after a joint VA OIG–US State Department investigation charged 12 individuals and 24 companies with thousands of false claims — services double-billed, overpriced, unnecessary, or never provided — an estimated US$67 million loss (GAO-25-107149). ~1,030 veterans in Panama were told to switch to non-suspended providers.
  • September 2026: VA has named and suspended ~50 FMP providers amid the ongoing DOJ investigation (Stars and Stripes); veterans report real difficulty finding practices willing to bill FMP.
  • Panama was FMP's #1 claims country 2018–2024 (220,968 paid claims — more than the Dominican Republic and Germany combined), partly fraud-inflated per GAO.
  • The fallout: a smaller provider pool, 4–6-month reimbursement delays, a June 30, 2026 House hearing on the proposed FMP Integrity and Improvement Act, $106 million recovered in VA–CMS duplicate billings, and the end of paper checks (September 30, 2025) — direct deposit is effectively mandatory.

Protect yourself:

  1. Check every provider against the suspension list on va.gov's FMP alerts page before treatment, and re-confirm direct billing with the billing office.
  2. Keep itemized receipts stamped "pagado" plus proof of payment; file promptly online, with direct deposit on.
  3. Ask the hospital to file the claim so VA pays it directly — not you floating thousands for months.
  4. Use the free claims help at American Legion PC-02's two VA-accredited VSOs — no membership required.
  5. Remember: suspensions target providers, not veterans — your FMP eligibility is intact.

What military conveniences do I lose — and how do I handle paperwork?

Gone since the December 31, 1999 handover: commissaries, exchanges, MWR, military hospitals, APO/FPO mail, Space-A terminals. The 2025 US–Panama MOU brought rotational troops back but zero retiree privileges — even 100% P&T veterans have no commissary access. Nearest Space-A terminal: Soto Cano AB, Honduras (retirees Category VI OCONUS; 100% P&T CONUS-only). Mail runs through a Miami freight-forwarder/PO box.

DD-214 and records: free via eVetRecs (vetrecs.archives.gov; ID.me works from abroad) or SF-180; the embassy's ACS unit can certify true copies.

Military ID / DEERS: ⚠️ no RAPIDS/DEERS site exists in Panama — renewals require a US trip (regional processing runs through Miami under SOUTHCOM; this gap is undocumented anywhere official). An expired ID disrupts TRICARE claims and hospital verification — renew on every US visit.

Which configuration fits my situation?

Scroll the table sideways to see every column

Profile Recommended stack Typical monthly health cost
Under-65 retiree TRICARE Select Overseas (enroll!) + Pensionado discounts for routine care + optional local membership (~$22/mo) + medevac membership ~$16–31 enrollment (amortized) + $50–150 cash care; worst year capped at $4,381
65+ retiree (TFL) Medicare A+B — never drop Part B ($202.90/mo) — + automatic TFL as primary payer + discounts + medevac $202.90 Part B + modest cash; worst year capped at $3,000 (Group A)
100% P&T veteran VA comp ($3,938.58/mo single, 2026) + FMP pays all service-connected care at direct-billing hospitals + TRICARE/cash for the rest; spouse may qualify for CHAMPVA SC care $0 up front where direct billing exists; non-SC care discounted cash or TRICARE cost-shares

Budget A — under-65 retired couple, Coronado: retired pay ~$3,200 + VA comp (50%) ~$1,133 ≈ $4,300/month in; living ~$2,200 (guide 12) + ~$150 health + ~$25 medevac → comfortable margin.

Budget B — 65+ couple, Boquete: only mandatory premium = 2 × Part B ($405.80/month); TFL $0; living $2,000–3,000 (guide 13) + ~$100 cash care; one hospitalization stays under the $3,000 cap.

Budget C — single 100% P&T veteran, Panama City: VA comp alone clears the Pensionado threshold nearly 4×; FMP direct-bills service-connected care; discounts cover the rest.

Every profile: hold a medevac membership (~$200–400/yr) — TRICARE evacuates only to the nearest adequate facility, never home by choice, and retirees pay up front and claim.

The 10 questions to ask before moving

  1. Is my VA rating permanent, or will I face future C&P exams? (Either works — VES examines in Panama.)
  2. Which conditions are service-connected — and does my FMP benefits letter list them all?
  3. Is my preferred hospital off the FMP suspension list — and does it still direct-bill?
  4. Am I turning 65 soon — is Medicare Part B set to start at least 2 months before my birthday month?
  5. Is my military ID valid well past my next US trip? (No DEERS site in Panama.)
  6. Do my Login.gov/ID.me accounts work with a foreign phone and address? (VA.gov now requires them.)
  7. Is IDD set up for DFAS, VA, and SSA — with the old US account kept open until the first deposit lands?
  8. Do I hold medevac coverage? TRICARE will not fly me home.
  9. Have I built the itemized-receipt habit — "pagado" stamp plus proof of payment every visit?
  10. Does my family know the embassy FBU (Panama-FBU@state.gov, 317-5000) handles death-benefit and burial claims?
