Panama's senior-care system works differently from the US or Canada: it is home-care-first. Families, not facilities, absorb elder care — which keeps the caregiver labor pool deep and affordable. Residential assisted living exists but is thin: a handful of private homes in Panama City, two small homes in Alto Boquete, charity homes in David, and nothing in Coronado or the beach corridor. Most expat retirees plan to age in place with hired caregivers costing a fraction of US prices. This guide maps every verified option, the licensing test to demand, and how to hire a caregiver legally.
What assisted-living facilities actually exist in Panama?

| Facility | Where | Monthly cost | Notes |
|---|---|---|---|
| Insignia Panama | Calle 49, El Cangrejo, Panama City | $2,500–4,500 | US-model senior living (operator runs 6 communities in Puerto Rico). Independent living, assisted living, and a dedicated "Seasons" memory-care program with certified staff, 24/7 certified nurses, medication management, controlled access, in-room emergency call. +507-6936-1708. |
| Villa Esperanza / Boquete Home & Care | Alto Boquete, Chiriquí | $1,200 (shared) – $1,600 (private) | Two small homes, 12–15 residents each. 24-hour care, 3 meals + snacks, medication administration, recreational therapy, laundry. Owner Dalisla Sanchez speaks English. +507-6678-1278 / 6505-3077. |
| Golden Paradise | Urb. Los Ángeles, Panama City | Day care from $45/day (M–F 8–5); overnight/weekends $65/day; residence on request | Since 2016, max ~20 residents, Alzheimer-specialized staff, weekly physician visits included. +507 388-4237. |
| Harmony House | Pueblo Nuevo/Hato Pintado, Panama City | Not published | Boutique dementia/Alzheimer's residence; 24/7 nursing, permanent and temporary/daytime stays, postoperative and palliative care. |
| Abundant Life Home Health Care | Bella Vista, Panama City + Las Lajas, Chame | Not published | Licensed residential geriatric care home plus in-home caregivers; Montessori-inspired memory care, "Café de la Memoria" dementia support, hospice support. |
⚠️ Pricing caveat: Insignia publishes no rate sheet; the $2,500–4,500 range comes from three independent expat/media sources (2025–2026), not the operator. Confirm current rates directly. The same applies to Harmony House and Abundant Life.
Chiriquí/David has only small charity-run homes (Santa Catalina and San Miguel Arcángel in David; Mi Dulce Mansión in Dolega; Dios es Amor in Tijeras) — Spanish-language, small-scale, not aimed at expats. Coronado and the Panamá Oeste beach strip have no dedicated assisted-living facility at all — coverage there is via home-care agencies (see below).
What about Generation Tower in Costa del Este?
Generation Tower is a real-estate product, not a care facility. It is an Empresas Bern condo-hotel in pre-sale (units from ~$185,500, est. completion ~2027) whose marketing includes an "Independent Senior Living Program" (48 units) with smartwatch monitoring and emergency buttons. It is not licensed assisted living, and pre-construction purchases carry documented risk in Panama. Treat it as an apartment purchase with senior-friendly features — nothing more.
Government asilos — why they are not a practical option for expats
State-run or subsidized homes (MIDES, CSS, municipalities) exist and cost $600–1,000/month after a socio-economic evaluation. But they require age 60+, permanent residency, a social-worker study proving lack of family support, medical and psychological evaluations — and staff are Spanish-only. They are designed for low-income Panamanian seniors, not for expats who can pay. Budget the private market or home care instead.
How do I check whether a residence is legitimate?
There is no US-style assisted-living license or public quality grading in Panama. The core legitimacy test is one document:
Demand the MIDES "permiso de operación" (operating permit). MIDES (Ministerio de Desarrollo Social) licenses casas hogares and day centers; MINSA adds a sanitary inspection, and Bomberos a fire inspection. MINSA's own inspector checklist literally asks: "¿Cuenta el centro con permiso de operación expedido por el MIDES?" A residence that cannot show you this permit is operating outside the system — walk away.
Also ask for: the MINSA sanitary certificate, nurse staffing ratio, staff police records (required for licensing), and pricing with care-level escalation in writing. There is no public database of inspection results, so in-person vetting is essential.
What do caregivers and nurses cost?
Scroll the table sideways to see every column
| Arrangement | Typical cost (2026) |
|---|---|
| Full-time home caregiver | $600–1,200/month (minimum ~$400) |
| Part-time caregiver | $25–50/day (community reports $50–75/day for 3 days/week) |
| Private 12-hour day shift | $30–35 per shift |
| Live-in professional nurse (Panama City) | $1,100–2,000/month |
| Agency-placed caregiver | $1,500–2,500/month |
| True 24/7 registered-nurse coverage (multi-shift) | up to ~$9,000/month |
| Physiotherapy at home | $50–60/session (packages to $30) |
The lesson in the last row: round-the-clock nursing quickly exceeds facility costs. The affordable norm is a mixed model — a caregiver at $600–1,200/month plus nurse visits from an agency for clinical tasks. Compare with US home-health-aide costs of $6,000–15,000/month: Panama's advantage is enormous at low-to-medium acuity, but narrows at the top.
Verified home-care agencies: Vitae Salud (hospitalization-at-home, palliative, 24/7 nursing), Domisalud (Calle 53, PC), Abundant Life (PC + Chame), Panama Home Care (based in Playa Coronado, bilingual doctors/nurses, serves Chame→Penonomé and El Valle), Clínica Express (PC + Panamá Oeste). Verify any placement agency's license at apps.mitradel.gob.pa/ConsultaAgencia.
How do I hire a caregiver LEGALLY?
Panama treats your caregiver as a domestic worker under the Labor Code, and expat employers are increasingly audited. The checklist:
- Written contract, registered at MITRADEL (Registro de Contratos). Verbal deals are illegal; a written contract enables the 2-week legal trial period.
- CSS affiliation within 6 business days of the start date (Ley 51/2005, art. 77), via the CSS domestic-employees section; monthly quotas via SIPE.
- Employer quota: 13.25% of salary (Ley 462 de 2025), plus 1.50% Seguro Educativo and a Riesgos Profesionales premium. ⚠️ The quota rises to 14.25% in March 2027 and 15.25% in March 2029. Employee share (9.75% + 1.25%) is withheld from pay. Budget +15–17% over salary all-in. One consumer site claims an 8.25% domestic-employer rate — treat 13.25% as the safe planning number.
- Minimum wage floor: B/.350/month in Region 1 (includes Panama City, David, Boquete, Chame, San Carlos/Coronado) or B/.320 in Region 2 (Decreto Ejecutivo 13, effective 16-Jan-2026). Market caregiver rates are well above this floor.
- Décimo tercer mes: a 13th month of pay in three installments (Apr 15 / Aug 15 / Dec 15), at special CSS rates.
- Also owed: paid vacation (1 day per 11 days worked), prima de antigüedad (~1 week/year), 30 days' notice after the trial period, and indemnización for unjustified dismissal. Live-in: food and lodging count as part of pay unless contracted otherwise; legal rest 9pm–6am plus one full day off weekly.
- Foreign caregivers need a domestic-worker visa (processed by you at Migración) or a MITRADEL work permit.
Non-compliance is expensive: fines, surcharges, and — worst — the employer bears all medical costs if an uninsured worker is injured in your home.
What about palliative care and hospice?
Better than expected:
- Public: Panama has a formal Programa Nacional de Cuidados Paliativos (MINSA Resolución 499/2010, grounded in Ley 68/2003), serving 4,500+ advanced-illness patients a year, including home-based care through regional MINSA/CSS teams. Euthanasia is prohibited.
- NGO: HOSPES Pro-Cuidado Paliativo (founded 1992, volunteer physicians/nurses) is the national palliative-care association.
- Boquete: the Boquete Hospice & Health Foundation (boquetehospice.org) offers free services regardless of nationality — medical-equipment loans, companionship, incontinence supplies, nutritional pantry, end-of-life support for Boquete and Potrerillos. Register early; it is a genuinely unique expat-community asset. It also fields paid professional nurses/caregivers.
- Private: Vitae (palliative home care), Harmony House (residential palliative), Abundant Life (hospice support).
Does adult day care exist?
Barely, but it is growing. Golden Paradise offers the best-documented private option (from $45/day, M–F 8am–5pm, meals and activities included; overnight $65). Harmony House offers daytime "temporary stays." Sol Adult Daycare announced a 2026 opening in Panama City — verify it is operating before relying on it. MIDES opened a public Casa de Día pilot in Juan Díaz in mid-2025 (expat access unverified). This is a nascent category; do not assume day care exists outside Panama City.
Is there a medical-alert pendant service like Life Alert?
No. No US-style monitored pendant/bracelet service operates in Panama. The practical substitutes:
- SEMM membership (from ~$30/month; family-of-4 plan advertised at $48/month) — 24/7 bilingual-friendly private ambulance dispatch, doctor house calls and video consults at no per-use cost within the coverage radius (Panama City metro). Call 366-0122. This is the closest functional equivalent.
- SINAPROC ALERTA app (official, free) — geo-located emergency alerts.
- 911 — operators are Spanish-only (see our safety guide).
- Cellular/GPS SOS watches that dial family directly, plus building concierge protocols and the strong senior WhatsApp check-in networks in Boquete and Coronado.
Can I buy long-term-care insurance for Panama?
No. There is no LTC insurance product sold in Panama, and MINSA/CSS do not cover assisted-living or residential-care costs; the Pensionado medical discounts do not apply to room and board. Senior care is a cash-flow expense — plan it into your retirement budget. The silver lining: market estimates put Panama senior living at roughly 40–60% of US prices (assisted living $2,800–4,500/month here vs. $5,000–6,500+ in the US; memory care $4,000–6,000 vs. $7,000–10,000), and home care at one-fifth to one-tenth of US rates.
Bottom line
- Facility care: Insignia (PC) and Villa Esperanza (Boquete) are the two verified expat-viable residences; demand the MIDES permiso everywhere.
- Default strategy: age in place with a legally hired caregiver (~$700–1,500/month all-in) plus agency nurse visits.
- Safety net: SEMM membership + SINAPROC ALERTA app + a bilingual emergency service.
- Dementia: real memory care exists only in Panama City (Insignia, Harmony House, Abundant Life, Golden Paradise); elsewhere, home care plus AFAPADEA (the Alzheimer family association, +507 6780-7283) support.
- Coronado/David: plan for home care — there are no facilities.
